At the Ann Frances Outreach Foundation, we are committed to meeting people where they are, offering support with compassion, dignity, and a belief in every person’s potential. Our goal is to bring hope for the future by connecting individuals and families with the resources they need to find stability, rebuild their lives, and move forward with confidence.

Each week we provide a warm, nutritious meal, a listening ear, and essential support such as clothing and hygiene essentials, first aid, and housing support.
Tuesdays at the Norristown Transit Station and Thursdays at the Days Inn in Pottstown.

We support individuals experiencing long‑term homelessness or barriers to stable housing through personalized, one‑on‑one guidance that provides tailored support and advocacy. Whether securing housing, connecting people with essential services, or offering a steady presence, the goal is to ensure every individual feels seen, valued, and supported at every stage of their journey.

We assist individuals and families with securing and maintaining housing through a variety of services including assistance with life/goal planning, budgeting, job search/securing employment, SNAP and insurance benefits application, advocacy, rental and housing assistance.

The Swede Street Community is a safe and welcoming affordable housing initiative developed by the Ann Frances Outreach Foundation in Norristown. These residences provide stable, affordable homes for individuals and families, offering not just shelter but a true sense of community, dignity, and hope.

During the summer months, we deliver fresh fruits and vegetables to seniors who face barriers to accessing nutritious groceries. With Chat & Chew, each delivery becomes a chance for conversation, laughter, and meaningful connection.

We help individuals experiencing housing instability secure safe, affordable rentals by guiding them through the search, application, and transition process. When funding allows, eligible clients can receive partial assistance with move‑in costs and we work with landlords to support long‑term housing success.

We operate a year-round Emergency Food Pantry, ensuring individuals and families have access to essential groceries during times of crisis when other food resources are out of reach.

Each December we are able to deliver toys for children along with a turkey and all the fixings for a holiday meal, helping families experience a festive and comforting holiday together.
